Below are some of the
forms that you may be requested to complete prior to participating
in your personal training programme.

N.B. The forms will be kept safe in my keeping but are
ultimately your property. All information that you provide
will be kept confidential.
Forms Required For Your Consultation:
Forms Required ONLY Prior to the Start of Your
Programme:
- Client – Trainer
Agreement:
This outlines what you expect from me as your trainer
and also what is required of you during the programme.

- Informed Consent:
This will be only used for certain clients. During
your consultation depending on your health, fitness
and personal
goals, I may discuss the need for a fitness evaluation
to create a baseline for you. The tests may be cardiovascular
or of muscular strength/endurance. Your agreement to
these tests will be required.
Other Forms:
- Food Diary:
This is designed to record your diet over a one week
period so that your habits can be identified and
your intake related to your energy levels and hunger
levels. [download]
Use this for your own observations or alternatively complete
your
diary, send it to me together with an outline of
your daily activities and I’ll
arrange to meet with you to discuss your diet. This is free to regular
Personal Training clients.
N.B. You will need to
print or photocopy the second page
several times according to your needs as 1 page accommodates 2 meals.
